Eno hammock.
Didn't fail, per-se, but if you're taller than 5'8" you're NOT getting a good night's sleep, no matter how you hang it.

Also not really a failure, but the people that Go-Lite recommend to do their warranty repair work suck. Out loud.
I had a branch fall through my SL5 & rather than fix it myself I sent it to the recommended repair shop. Honestly, it looks like it was patched by a 7 year old. I mean, yeah, they fixed the tear but they left so much excess black tape material (I was expecting at least, color matched silnylon) it's not even funny. Can't complain at the price, they did do it for free, but it actually looked better with duct tape.

exped SYN mat UL7 mat. blown baffles, TWICE. the original and the warranty replacement. have since moved to thermarest xtherm.

plastic lightmyfire spork, twice. since got the Ti version.

My Kuiu Yukon rain gear failed epically while guiding in the Yukon this season. Hoping they'll warranty it, but I'll have a tough time trusting it now.
