I decided I better stop tempting fate, and bought a backup release. The first picture is my old release. You can see the metal loop that goes through the base of the release bends up on one side and down on the other, locking the release in place. I wanted to fire a few shots with the new release and the little rubber cover on the end of the metal loop came off it my hand. That was when I noticed the loop bent up on one side, but was circular on the other. The base on one hole just slides right off. What the eff? Quality control issue? I will beat the smart asses to the punch and let you know it is not easy to bend with hand tools.

Slide it back in and bend it. Problem solved
I had an attempted bending session fail before posting. After putting on my big boy pants and applying some elbow grease I got her bent. Still seems strange I had to do that. Oh, well. Case closed.

I have two of these releases. One I just recently bought to be my back up.

Your back up seems to not be bent to the angle/radius I have on my two examples, which is probably why it slipped out. Also, my newer release (the back up) never came with the rubber end cap.
