Trip/rescue insurance?

For those of you that are using trip or travel insurance, what companies do you recommend. We did an expensive trip this spring that I didn’t insure, but I have trips to Argentina, Africa and Alaska upcoming in the next 18 months and some will involve the entire family. They are all remote and expensive and I need recommendations on whom to use and not to use for both travel and medical rescue insurance. Any and all input is appreciated!!

Next time I go to New Zealand I'll use Global rescue. Not so much because I expect to need rescue but simply for the fact they offer travel insurance for hunting. I can not find a standard travel insurance policy that covers hunting. Hunting is always on the list of excluded activities.
